A technically strong plan can fail because the right person was not consulted, a decision was not understood, a team was overloaded, or a stakeholder heard important news too late.
Communication is a design problem

Team development
Teams evolve. New teams often need more structure and clarity. Experienced teams may need more autonomy and fewer unnecessary approvals. The PM should adapt leadership to the team rather than applying one management style everywhere.
Escalation without drama
A good escalation states the situation, impact, options, recommendation and decision required. “We have a problem” is less useful than “The approval is three days late; it threatens the integration test window; here are two recovery options; I recommend option A.”
